Load the app.php before running apps' boot method

Some apps require the composer autoloader from app.php. If we run boot
before including that file, classes and functions from dependencies

Add a message to the log entry of an app being disabled
...
As an admin, it's always a surprise to see that an app got disabled. On
a busy server with many log entries, it's hard to locate the entry that
explains why Nextcloud disabled an app. Adding a message will make it
more obvious, allowing admins and developers to grep for the string.

To continue this formatting madness, here's a tiny patch that adds
unified formatting for control structures like if and loops as well as
classes, their methods and anonymous functions. This basically forces
the constructs to start on the same line. This is not exactly what PSR2
wants, but I think we can have a few exceptions with "our" style. The
starting of braces on the same line is pracrically standard for our
code.
This also removes and empty lines from method/function bodies at the
beginning and end.

optimize batch generation of previews

by allowing the generation of multiple previews at once we save on having to find, open and decode the max-preview for every preview of the same file
the main use case for this is the preview generator app (pr for that comming next)
in my local testing this saves about 25% of time when using the preview generator app

Without this patch the hook does not transport the information whether the login is
done with an app password or not. The suspicious login app requires the parameter
to function correctly, hence adding it will make suspicious login detection also possible
with SAML users.

If `is_writable()` fails, fall back to logic that attempts to create a file
and then checks if it exists. If this check fails, an error occurs as it
did before.

We should get the version of the app. Not of the appfolder. Else there
is no way to properly compare the versions.
Now note that installing in 1 go will still fail. But at least on the
next page load the new version will be properly detected.

Don't lie about the preview mimetype
...
For legacy reasons we stored all the previews with a png extention.
However we did not put png data in them all the time.
This caused the preview endpoints to always report that a preview is a
png file. Which was a lie.
Since we abstract away from the storage etc in the previewmanager. There
is no need anymore to store them as .png files and instead we can use
the actual file extention.

Properly allow \OCP\Authentication\IApacheBackend to specify logout URL

Any `\OCP\Authentication\IApacheBackend` previously had to implement `getLogoutAttribute` which returns a string.
This string is directly injected into the logout `<a>` tag, so returning something like `href="foo"` would result
in `<a href="foo">`.
This is rather error prone and also in Nextcloud 12 broken as the logout entry has been moved with
054e161eb5nextcloud/user_saml#112 ,
people cannot logout anymore when using SAML using SLO. Basically in case of SAML you have a SLO url which redirects
you to the IdP and properly logs you out there as well.
Instead of monkey patching the Navigation manager I decided to instead change `\OCP\Authentication\IApacheBackend` to
use `\OCP\Authentication\IApacheBackend::getLogoutUrl` instead where it can return a string with the appropriate logout
URL. Since this functionality is only prominently used in the SAML plugin. Any custom app would need a small change but
I'm not aware of any and there's simply no way to fix this properly otherwise.

This code is a leftover from the old Shibboleth implementation from ownCloud days, nowadays it causes more issues than it does good (such as https://github.com/nextcloud/user_saml/pull/137 ). Let's remove it for the future thus.

Move the inline <script> after the external <script> because the internal JS between CSS <link> and external JS makes firefox loading CSS and JS not in parallel.
The internal js is now parsed last but will still be executed first since everything else is deferred js.
